-          inspect and copy your information

-          amend information you feel is incorrect

-          request an accounting of disclosures of your information

-          request restrictions of disclosures

-          request confidential communication

-          have a paper copy of our complete notice of privacy practices

 We reserve the right to change this notice.

 You may file a complaint without penalty with us or with the Secretary of the Department of Health and Human Services if you feel your privacy rights have been violated.
